Monthly Meeting Report
Instructions:
This report should be written by the tutor and learner TOGETHER and completed by the end of the month. It is recommended that tutors and learners have a conversation about the questions below and complete this form together during their last tutoring session of the month. 

FOR QUESTIONS 9-13: It is recommended that tutors pose these questions to the learner and type out the learner's response. Tutors can and should add any information that can help answer the question.

Only 1 form per tutor & learner pair.

This form is due by the 10th of each month.

(Example response: "My supervisor complimented me on my improved speaking and I also understand the news on the radio better. I still need more practice pronouncing and spelling some words. Learner also needs to practice enunciating last letter of some words.")

(Example response: "I want to practice with reports that I need to write for work and improve my emails. I still have trouble writing in long sentences in emails.")
