Medicare Savings Program & Medicaid Application Assistance for the 65+, Blind and Disabled Population Wednesday, April 9, 9:30 am - 5:00 pm Did you know that you can save $2200 or more a year with the Medicare Savings Program? Health & Welfare Council of Long Island’s certified Healthcare Facilitated Enrollers will be at the library to provide enrollment assistance.The Facilitated enroller will help clients fill out all applications and collect required documents to submit to the Department of Social Services. Sign Up to be a Literacy Volunteer Are you interested in helping an English speaker learn to read and write? The Library is looking for volunteers with instructional experience. You can open countless doors for someone else by offering the gift of literacy. Interested persons should complete this application and contact Jack in the Reference Department at 631-286-0818 ext 1.

The Library has a Notary Public Fee: $2.00 per notarized signature We are not able to notarize wills, mortgages, or sales of property. All documents must be in English. You must have a valid New York State driver’s license or identification card. You may use a valid United States passport but must also have proof of residence that shows your current address. No post office boxes.

Passport Services by Appointment We serve first-time applicants, minor applicants, adult applicants whose most recent passport was issued as a minor, and applicants who have lost their passports.
|

Hours are by appointment only. You may also call us at 631-286-0818 x1. Our service cannot renew a passport. That is a separate process: Printed forms are available by the Reference Desk.
