Come to the Library to get your Flu Shot! Tuesday, September 15, 11:00 AM - 2:00 PM We've partnered with CVS to hold a walk-up flu-shot clinic in our parking lot. Masks are required and social distance must be maintained. If able, please print and complete this form before coming to the clinic (copies will also be available on-site). For questions, please contact CVS Pharmacy at 631-654-0329. Clinic is available to those who have health insurance. If you do not have health insurance, we encourage you to visit your local CVS or other pharmacy to pay for a flu shot ($40-50 per regular dose / $70-80 per high dose for seniors).
